5. Provide details on what security measures you intend to use.
6. Explain the intended use of your crops and how you intend to dispose of excess plant material.
7. Pay the licensing fee.

Once you obtain your license, you will also be required to post signage at your farm and submit additional reports to the Utah Department of Agriculture and Food. There are also specific requirements for harvesting and testing.
